Equity Bank reserves the exclusive right to determine whether to use any submitted image and maintains the sole discretion to reject any image for any reason whatsoever or without assigning a reason. Further, if images are deemed unacceptable by Equity Bank and are repeatedly submitted, Equity Bank reserves the right to prohibit future use of this service.

Prohibited images and subject matter include but are not limited to: copyrighted material, trademarks, branded products/services, or any company images not licensed to or owned by the cardholder. Equity Bank also reserves the right to reject any images deemed inappropriate or offensive in subject matter and Equity Bank reserves the right to determine what images are classified as inappropriate or offensive.

By accepting these terms and by submitting your images, you agree with the Equity Bank My Debit Card Design guidelines as follows:

• You certify that you have read, understood, and are following our image guidelines.
• You represent and certify that you have the sole rights to use the submitted images and that you have obtained any necessary consent to allow Equity Bank and its suppliers or contractors to alter, copy, print, distribute, prepare, and use the image (or an image derived from the submitted image).
• You hereby indemnify and hold harmless Equity Bank and its suppliers and contractors from and against all claims that may arise from your use of this service or the subsequent inclusion of the submitted image (or an image derived from the submitted image). This includes any liability incurred due to any third party action which asserts that a photograph or other document provided for use violates any claimed copyright trademark or other intellectual property rights of that third party.
• A design fee of $5.00 applies for the use of this service and will be deducted from your account associated with your card upon and subject to Equity Bank approving and printing your card. This design service fee applies to every card with an accepted image whether it is for a new or existing card, including replacement cards. Other fees many apply in addition to the card design fee. See your local Equity Bank branch for details.
• These terms relate only to the custom debit card design service (My Debit Card Design). Additional general terms and conditions apply to your card or accounts with Equity Bank. Terms and conditions may vary by Equity Bank location.
• Once your custom card is printed, you may pick it up at any Equity Bank location. Proper identification must be presented and verified in order to receive your new custom card at the bank.
• If your card has expired, is lost or stolen, or if you merely wish to change the picture you may order a new customer debit card as a replacement. You will need to create a new card design and submit it for approval. Design fees and standard replacement fees will apply.
• The replacement image must adhere to our image guidelines as well as these terms and conditions. If an image is rejected, you will be notified to submit another image.
• Equity Bank reserves the right to refuse any image or to terminate this service at any time.
